12. Form of the warrant. ___ (1) The warrant shall be issued ordinarily in the manner prescribed in schedule IV: Provided that the warrant may also be issued in a manner proposed by the authorized officer under Section 8(b) with or without modifications by the Judge. (2) The warrant shall be signed by the Judge and a seal of the Court shall be affixed.
